We start with a thorough evaluation of the aging client. The assessment includes a review of the client's medical condition and cognitive abilities, a medications review, a safety evaluation and assessment of emotional and social needs.
Care Plan
We work with the client, their family and other healthcare professionals to develop a comprehensive, personalized and cost effective care plan. We not only look at the current situation, but also help the family with long term planning and financial considerations. This plan is designed to provide the client with the highest quality care while preserving their independence and dignity.
Arranging Caregivers and Other Service Providers
When necessary we can arrange (on your behalf) for caregivers and any other service providers required to meet the needs of the client. You or your designee will contract for these servicers and be billed directly. We will monitor in-home help or other services. We can also provide caregiver coaching to assure the highest level of care for your loved one.
Liaison Between Health Care Providers
Aging clients often have multiple physicians involved in their care. These physicians will not necessarily be aware of each other's involvement. After you have signed the appropriate releases we can promote communication between the physicians with medication updates and reporting any changes in the health of the elderly client. GCC can take the aging client to appointments to ensure clear communication between the client and the doctor and then be in a position to provide an accurate report back to the family.
Communication
We regularly update the family members as to the status of our efforts and share any concerns that may arise. We will make recommendations for changes in approach as required.
